IMPORTANT INFORMATION - PLEASE READ COMPLETELY before submitting the form. Health Requirements: All dogs must have current vaccinations. All dogs and puppies must have been in your home without illness for the two weeks prior to class. Training Services Provided: I understand that the dog trainers at the School of the Naked Dog, will work directly with me and my animal companion to impart contemporary animal behaviour knowledge that best fits our needs, and that successful companion pet programmes depend on a combination of learned skills on the part of the pet and the owner. Trainers will make every reasonable effort to help us attain goals, but makes no guarantees of performance on the part of Owner or pet as a result of providing professional animal behaviour consultation. Trainers, at their discretion, may refuse entry to a pet which is not healthy, is aggressive, or does not seem suitable for the services provided. Waiver, Assumption of Risk, and Agreement to Hold Harmless: I understand that participation in dog training is not without risk to myself and my dog as well as any family members or guests who might attend. I hereby waive and release Natalie Perzylo and Blair Anderson, and their Agents, and the owner/lessor of the facility from any and all liability of any nature, for injury or damage suffered as a result of my participation in any class or private lesson, including any damage or injury resulting from the action of any person or dog, or for any accident occurring on the premises or its surroundings. I agree to accept full responsibility for my actions, for the actions of my dog, and the actions of my family or guests. Payment: You need to prepay in order to secure your place in our class.
